Special Needs plans (SNPs) are a type of Medicare Advantage plan intended for people diagnosed with select conditions or diseases, or folks with low incomes. It can be difficult to have a condition that requires a lot of healthcare and equally hard to pay for this care on a fixed or low income. You can join a special needs plan through us after you’ve enrolled in Medicare Part A and Part B. Our SNPs cover your Original Medicare benefits and also include coverage for dental, vision, and hearing, plus prescription drugs. Three Medicare Special Needs Plans are available: Chronic conditions (C-SNP), Dual-eligible (D-SNP), and Institutional (I-SNP). This post will focus on D-SNP.

Who Is Eligible for a Dual-Eligible SNP?

You can enroll if you’re eligible for Medicare Part A and enrolled in Part B AND Florida Medicaid. To be eligible for Medicare, you must be 65 or older, or have specific health conditions.

Florida Medicaid is the state’s Medicaid program, which is based on income levels — not age. You can be under 65 if you meet the income requirements.

Your Medicare agent will verify your eligibility for Medicaid before enrolling you in a Dual-eligible special needs plan.

When to Enroll

You can enroll in a Medicare Special Needs Plan during your Initial Enrollment Period (IEP) — when you first become eligible for Medicare at age 65, or during the Annual Enrollment Period (AEP) from October 15 to December 7 each fall.

In certain situations, some individuals may also qualify for Special Enrollment Periods (SEPs) during the year. These situations include:

  • Qualifying for Medicaid
  • Moving out of state or outside of your current plan’s coverage area
  • Moving into, living in, or moving out of an institution
  • Being diagnosed with a chronic or severe condition that makes you eligible for a Chronic Condition SNP

If you lose your eligibility for a Special Needs Plan, you’ll have a “grace period” that permits you to leave the plan and join another, or go back to Original Medicare.
